Colchicine proves 'safe and effective' in the prevention of recurrent pericarditis, trial finds

ScienceDaily (Aug. 28, 2011) ? Colchicine, when given in addition to conventional therapy, was more effective than placebo in reducing the incidence of recurrence and the persistence of symptoms of pericarditis in a randomised controlled trial. This is the first time that the efficacy of colchicine in preventing recurrent episodes of pericarditis has been demonstrated in a double-blind multicentre randomised trial.

"Recurrence," said investigator Dr Massimo Imazio from the Maria Vittoria Hospital in Turin, Italy,"is the most common complication of pericarditis, affecting between 20 and 50% of patients. Recurrences can be frequent and may seriously affect quality of life, cause hospital readmission, and increase management costs. There has been some preliminary data from non-randomised observational studies and two single-centre open-label randomised studies suggesting that colchicine may be a safe and useful drug for preventing these recurrences. Our aim was to test these suggestions in a multicentre double-blind randomised placebo-controlled trial."

Indeed, it was on the basis of such non-randomised observational findings (as well as expert opinion) that colchicine was recommended for the treatment of recurrent pericarditis (class I recommendation) in the 2004 guidelines on pericardial diseases of the European Society of Cardiology.

The CORP trial, an independent non-sponsored study and the first multicentre double-blind randomised trial of colchicine in the secondary prevention of pericarditis, was performed in four centres in Italy and recruited 120 consecutive patients with a first episode of recurrent pericarditis. The primary endpoint of the study was the recurrence rate at 18 months; the secondary endpoints were symptom persistence at 72 hours, remission rate at one week, the number of recurrences, time to first recurrence, disease-related hospitalisation, cardiac tamponade, and constrictive pericarditis rates.

Colchicine was given as adjunctive therapy at an initial dose of 1.0-2.0 mg for the first day and a maintenance dose of 0.5-1.0 mg daily for the following six months. The lower dose (initial dose: 0.5 mg every 12 hours and maintenance dose 0.5 mg daily) was given to patients under 70 kg in weight or intolerant of the highest dose (initial dose 1.0 mg every 12 hours and maintenance dose of 0.5 mg every 12 hours).

Results showed that colchicine significantly reduced the incidence of recurrences at 18 months when compared to placebo (24% vs. 55%, p<0.001). In addition, symptom persistence at 72 hours was significantly lower in the colchicine group than in the placebo group (23.3% vs. 53.3%, p=0.001) as were the mean number of recurring episodes. The rate of remission at one week was higher in those patients given colchicine than placebo (82% vs. 48%; p<0.001), as was the time to a subsequent recurrence. The rate of side effects was similar in the colchicine and placebo groups (7% vs. 7%; p>0.99).

Commenting on the results, Dr Imazio said: "When added to empiric anti-inflammatory therapy, colchicine appears to be a safe low-cost drug for rapid symptom relief, improved remission rates at one week, and reduced recurrence after an initial episode of recurrent pericarditis.

"However, our findings might not be generalisable to other settings or other patient populations. This trial only addressed the use of colchicine following a first recurrence of pericarditis, and not in patients with multiple recurrences. We recruited only adult patients, and thus cannot apply our results to paediatric populations. We excluded patients with bacterial or neoplastic pericarditis, patients with transaminases elevation or severe liver disease, elevated creatinine, patients with myopathy, blood dyscrasias or gastrointestinal disease, pregnant and lactating women, as well as women of childbearing potential and not using contraception.

"It should also be noted that colchicine is not registered for the prevention of pericarditis and its use for this indication is off-label."

JMAYL, Libya (Reuters) ? Libyan rebels swept into the town of Jmayl on Saturday, consolidating their grip on areas to the west of Tripoli after forces loyal to Muammar Gaddafi fled the home base of Libya's prime minister.

Gaddafi supporters are understood to have fled to the town as rebel forces rolled up areas to the south in Libya's Western Mountains in a campaign that eventually took them all the way to Tripoli.

Jmayl was one of a number of pockets of Gaddafi loyalists that remain in parts of Libya despite the rebel advance.

The two sides had been negotiating for days in an attempt to end the stand-off over Jmayl and arrange prisoner swaps, but talks had foundered in an atmosphere of mistrust.

"There were soldiers in the town and supporters of Gaddafi. We spread a rumor three days ago during the talks that we would free Jmayl today," Zuwareh said.

Rebels said Jmayl had been a stronghold for Gaddafi loyalists in the west and was the hometown of Libyan prime minister Al Baghdadi Ali al-Mahmoudi, who recently left the country and was seen five days ago on the southern Tunisian island of Djerba.

The rebels said the Gaddafi loyalists had fled Jmayl, a small dusty desert settlement whose inhabitants welcomed the rebels with V-for-victory signs.

At the town hospital, two wounded rebel fighters were brought in for treatment, along with one man who had been held prisoner by the Gaddafi forces before they left.

One fighter described the climate of suspicion that clouded negotiations.

"Yesterday there was a Gaddafi loyalist in his car ahead of us. We shouted at him to stop and put down his weapon," said Mohammed al-Mansouri, a 23-year-old fighter.

"He got out and made like he was putting it down, then he suddenly picked it up and sprayed us with gunfire.

"So we had to shoot him dead."

Japan nuke plant radiation leak exceeds Hiroshima (AP)

TOKYO ? The amount of radioactive cesium that has leaked from a tsunami-hit nuclear plant is about equal to 168 of the atomic bombs dropped on Hiroshima at the end of World War II, Japan's nuclear agency said Friday.

The Nuclear and Industrial Safety Agency supplied the estimate at a parliamentary panel's request, but it noted a simple comparison between an instantaneous bomb blast and long-term accidental leak is impossible and the results could be "irrelevant."

The report estimated for each of the 16 isotopes released from "Little Boy" and 31 of those detected at the Fukushima plant but didn't provide the total. NISA has said the radiation leaked from Fukushima was about one-sixth of what the Chernobyl disaster released in 1986.

The March 11 earthquake and tsunami caused three reactor cores at Fukushima Dai-ichi nuclear power plant to melt. Several blasts and fires also sent massive radiation into the environment.

The report said the damaged plant has released 15,000 tera becquerels of cesium-137, which lingers for decades and could cause cancer, compared with the 89 tera becquerels released by the U.S. uranium bomb.

The bomb "Little Boy" dropped on Aug. 6, 1945, destroyed most of the city and killed as many as 140,000 people. A second atomic bombing three days later in Nagasaki killed tens of thousands more, prompting Japan to surrender and ending WWII.

The Hiroshima bomb claimed most of its victims in the intense heatwave and neutron rays from a midair nuclear explosion and the highly radioactive fallout. No one has died from radiation leaks from the Fukushima plant, where explosion from hydrogen buildup damaged reactor buildings but did not involve reactor cores.

The report estimated that Iodine-131, another isotope that accumulates in thyroid gland, and Stronthium-90, which has a 28-year half-life and could accumulate in bones, leaked from the plant in amounts about equal to 2.5 of the Hiroshima bombs.

A separate government report released Thursday said that 22 percent of Cesium-137 and 13 percent of Iodine-131 released from the plant during the crisis have fallen on the ground, with the remaining either fell into the ocean or outside the area of simulation.

The National Institute for Environmental Studies said its simulation of aerial flow, diffusion and deposition of the two isotopes released from the tsunami-hit plant showed their impact reached most of Japan's eastern half, ranging from Iwate in the north to Tokyo and central prefecture (state) of Shizuoka. Both Iwate and Shizuoka are more than 180 miles (300 kilometers) away from the plant.

The study also showed that Iodine-131 tended to spread radially and Cesium-137 tended to create "hot spots."

Some 100,000 people evacuated their homes due to radiation threats from the Fukushima plant.

On Friday, about 30 residents from the immediate neighborhood of the plant were allowed to briefly return home to get clothes and other necessities they left early in the crisis. But officials have said their area may stay off-limits for years.

Plant operator Tokyo Electric Power Co. and the government aim to bring the reactors to stable cold shutdowns by early January. The government is working to decontaminate areas outside the 12-mile (20-kilometer) restricted zone where access may be relaxed in coming weeks.

Lawyers wrap up Int'l Court's first trial (AP)

THE HAGUE, Netherlands ? Prosecutors began wrapping up the International Criminal Court's landmark first trial on Thursday by urging judges to convict a Congolese warlord of recruiting hundreds of child soldiers and sending them to fight and kill in his country's brutal conflict.

Deputy Prosecutor Fatou Bensouda told judges that evidence in the trial that began in January 2009 gave voice to children that militia leader Thomas Lubanga had "transformed into killers; those girls that Mr. Lubanga offered to his commanders as sexual slaves."

Bensouda said the armed wing of Lubanga's Union of Congolese Patriots political party trained hundreds of children in 20 camps scattered across the Ituri region of eastern Congo in 2002-2003.

"They were used to fight in conflicts. They were used to kill, rape and pillage," she added.

Lubanga's defense lawyers are expected to tell judges on Friday that the prosecution evidence was flawed by false witness testimony and that Lubanga in fact tried to liberate child soldiers, not recruit them.

Lubanga's trial has been hailed as a significant step in the development of international law. It was the first international case to focus exclusively on child soldiers and the opening trial at the world's first permanent war crimes tribunal.

However, it also was overshadowed by delays and by friction between prosecutors and judges.

The trial was put on hold in June 2008 ? just 10 days before it was scheduled to start ? when judges ruled that Prosecutor Luis Moreno-Ocampo had not given lawyers evidence that could have helped Lubanga. He refused to turn over some 200 documents because they came from organizations including the United Nations on condition that they not be disclosed to others.

Judges said the confidentiality agreements meant, "the trial process has been ruptured to such a degree that it is now impossible to piece together the constituent elements of a fair trial."

Moreno-Ocampo eventually got consent from all the organizations and disclosed the material to defense attorneys, allowing the trial to get under way.

But judges halted it in July 2010 and ordered Lubanga released from custody when prosecutors defied a court order to reveal the identity of an intermediary who had helped prosecutors contact witnesses.

Prosecutors appealed the decision and Lubanga remained in his cell, but the incident underscored simmering tensions between prosecutors and the judges over disclosure of sensitive evidence.

Faced with the prospect of the case collapsing, prosecutors revealed the identity of their intermediary to the defense and appeals judges ruled that the trial could resume.

The tensions resurfaced Thursday when Presiding Judge Adrian Fulford refused to let Moreno-Ocampo answer a question addressed to one of his team of lawyers.

"Mr. Ocampo, can we please have some order," Fulford said. When Moreno-Ocampo tried again to answer, Fulford sternly told the prosecutor: "Mr. Ocampo, not at the moment."

Lubanga was arrested in March 2006, the first suspect to come into the custody of the International Criminal Court, which became operational in 2002.

The court has since issued indictments in high profile flashpoints such as the Darfur conflict in Sudan and the Gadhafi regime's brutal but unsuccessful campaign to stamp out dissent.

Lubanga is accused of leading a rebel group called the Union of Congolese Patriots which used child soldiers in savage fighting in the Ituri province in 2002-2003.

While welcoming his prosecution and the groundbreaking child soldier charges, human rights groups have criticized prosecutors for the narrow scope of the trial, saying they should also have charged him for the numerous rapes that victims say members of his militia perpetrated in a region notorious for widespread sexual violence.

"It was shocking to many of us that the announcement in 2006 of the case against Mr. Lubanga did not include charges for such crimes and overlooked the suffering of thousands of women," said Brigid Inder, executive director of a group called Women's Initiatives for Gender Justice.

However others applauded the likely deterrent effect of the Lubanga trial.

The U.N. special envoy for children in armed conflicts, Radhika Coomaraswamy, said leaders in conflict zones have often asked her about the ICC and the Lubanga prosecution.

"I found that fear of the ICC a healthy development in international law," Coomaraswamy told The Associated Press. "Nobody can measure how many children have been saved because of deterrence. That's not something you can measure, but hopefully that will be the case."
